Minor Policy & Parental Consent

Effective Date: May 16, 2026

1. MINOR PARTICIPATION
Individuals under eighteen (18) years old may only participate in Recovery+ services with written consent from a parent or legal guardian.

2. PARENTAL RESPONSIBILITY
Parents/guardians accept full responsibility for the minor’s participation, health disclosures, conduct, and compliance with facility policies.

3. AGE RESTRICTIONS
Certain modalities may require minimum age restrictions or additional medical clearance, including:
- IV therapy
- Sauna
- Cold immersion
- Oxygen therapy
- Cryotherapy
- TENS therapy

4. REQUIRED SUPERVISION
Children under sixteen (16) years old must remain supervised by a parent or legal guardian unless participating in an approved supervised service.

5. FACILITY SAFETY
Running, horseplay, misuse of equipment, unsupervised children, or disruptive behavior is prohibited.

6. MEDICAL DISCLOSURES
Parents/guardians must disclose:
- Allergies
- Medical conditions
- Medications
- Prior injuries
- Neurological conditions
- Cardiovascular conditions

7. RIGHT TO REFUSE SERVICE
Recovery+ reserves the right to refuse or discontinue services to minors at any time for safety reasons.

8. RELEASE OF LIABILITY
Parents/guardians agree to release Recovery+ from liability associated with minor participation except in cases of gross negligence or intentional misconduct.
